Volunteer Application

For volunteer roles that take place on the school campus, volunteers will need to go through the San Diego Unified School District's (SDUSD) application and screening process.

Volunteers will need to provide the following documents to the School Office

• Application (completed)

• Code of Conduct (completed)

• Copy of Drivers License, or CA  Identification Card

• Copy of Covid19 Vaccine Card (If not vaccinated, volunteers will need to show a negative Covid test taken 48 hours prior the start of each volunteer activity.)

In order to reduce spread of Tuberculosis (TB), SDUSD assess a volunteers risk of TB using the questionnaire  below.  The questionnaire must be administered by a licensed health care provider (physician, physician assistant, nurse, or nurse practitioner) prior to any volunteering.  The Adams Elementary Nurse can administer the questionnaire.  The health care provider will evaluate the questionnaire and ask follow-up questions as necessary.  If the provider deems it necessary, the volunteer will need to get a TB test with a negative result returned to the office prior to any volunteering.
